Understanding the Condition
WHAT IS LUMBAR STENOSIS?
Lumbar spinal stenosis is a narrowing of the spinal canal that creates pressure on the nerves traveling to the legs. It typically develops gradually from disc degeneration, facet arthritis, and ligament thickening. The hallmark symptom is neurogenic claudication — leg pain, heaviness, or cramping with walking that is relieved by sitting or bending forward.
Central Stenosis: Narrowing of the central spinal canal. Produces bilateral leg symptoms, worse with walking and standing, better with sitting or flexion.
Foraminal Stenosis: Narrowing of the nerve exit points (foramina). Produces more unilateral, radicular-type symptoms. Often responds well to specific manual therapy techniques.
Degenerative Spondylolisthesis: One vertebra sliding forward on another, contributing to canal narrowing. PT is the first-line treatment before surgical consideration.
Stenosis With Neurogenic Claudication: The classic presentation — limited walking tolerance with relief in flexion. Flexion-based PT, core stability, and manual therapy are highly effective.
