Understanding the Condition
SPINAL FUSION REHAB
Spinal fusion surgically joins two or more vertebrae together to eliminate motion at that segment — typically for instability, stenosis, spondylolisthesis, or failed disc management. Bone fusion takes 3–6 months. During that time, the segments above and below the fusion must be protected from excessive stress.
Lumbar Fusion: L4-L5 and L5-S1 are the most common levels. PT focuses on protecting the fusion, restoring lumbar mobility above and below the fused level, and building core stability.
Cervical Fusion: Following disc herniation or stenosis surgery. PT addresses cervical mobility at non-fused segments, deep neck flexor strengthening, and postural correction.
Multi-Level Fusion: More extensive surgery with longer timelines and greater functional demands on the surrounding segments.
Adjacent Segment Syndrome: Increased stress on the segments above and below a fusion over time. PT is the primary prevention and management strategy.

Post-fusion rehab must balance protecting the fusion during bone healing with maintaining and building the mobility and strength that the fused segments can no longer provide.

Protection & Early Mobility (Weeks 1–6)
Walking program, posture education, and gentle mobility work at non-fused segments. No bending, lifting, or twisting beyond surgical parameters.

Core Stabilization (Weeks 6–16)
Deep stabilizer activation and progressive core strengthening — building the support system that protects the fusion long-term.
03
Return to Full Function
Progressive loading, lifting mechanics, and the strength and movement program that maximizes long-term outcomes and minimizes adjacent segment stress.
Typical TimelineReturn to light activity: 3 months. Full recovery: 6–12 months depending on fusion level and extent.
