Understanding the Condition
WHAT IS SCIATICA?
Sciatica refers to pain that travels along the path of the sciatic nerve — from the lower back through the hip, buttock, and down the leg. It's a symptom, not a diagnosis. The actual cause can be a herniated disc, spinal stenosis, piriformis syndrome, or SI joint dysfunction — and the treatment depends entirely on which one.
Disc-Related Sciatica: A herniated or bulging lumbar disc pressing on a nerve root. Most responsive to directional movement therapy, manual therapy, and specific loading strategies.
Piriformis Syndrome: The piriformis muscle in the buttock compressing the sciatic nerve. Responds extremely well to manual therapy, stretching, and hip strengthening.
Spinal Stenosis: Narrowing of the spinal canal creating nerve compression. Often presents with bilateral symptoms, worse with walking. Flexion-based PT is typically most effective.
SI Joint Dysfunction: Sacroiliac joint irritation referring pain into the buttock and leg. Often misdiagnosed as disc sciatica — requires different treatment.

Movement testing, nerve tension assessment, and manual examination to determine whether this is disc, piriformis, stenosis, or SI joint — because each requires a different approach.

Decompress the Nerve
Manual therapy, joint mobilization, directional movement therapy, and soft tissue work targeted at the specific structure that's compressing the nerve.

Strengthen & Prevent Recurrence
Core stability, hip strength, and the postural and movement habits that take load off the lumbar spine and prevent this from coming back.
Typical TimelineMost patients see significant improvement within 4–8 weeks. Severe or long-standing sciatica may take 10–16 weeks.
