WHAT IS CHRONIC LOW BACK PAIN?
Chronic low back pain is defined as pain persisting beyond 12 weeks. The majority of cases are mechanical in origin — driven by specific structures including discs, facet joints, the SI joint, or surrounding musculature — rather than being truly non-specific. Finding the driver is the treatment.
Disc-Driven Low Back Pain: Disc bulge, herniation, or degeneration loading pain-sensitive structures. Responds well to directional movement therapy and spinal stabilization.
Facet Joint Pain: The small posterior joints of the spine becoming irritated from sustained positions, extension loading, or degeneration. Responds very well to manual therapy and mobility work.
Muscle-Driven Low Back Pain: Chronic muscle guarding, weakness, and compensation patterns that have developed around an original injury. Often the last thing to resolve without targeted intervention.
Movement Pattern-Driven Pain: How you move — bending, lifting, sitting — is continuously loading a sensitized structure. Correcting the movement pattern is the fix.
