Understanding the Condition
WHAT ARE CERVICOGENIC HEADACHES?
Cervicogenic headaches originate from the cervical spine — most commonly the upper three levels (C0-C1, C1-C2, C2-C3) — and refer pain into the head via the trigeminocervical nucleus. They are frequently misdiagnosed as migraines or tension headaches and treated ineffectively for years.
Classic Cervicogenic Headache: Unilateral head pain starting at the base of the skull, often with restricted neck mobility on the ipsilateral side. Responds very well to upper cervical manual therapy.
Sub-Occipital Headache: Tightness and pain at the base of the skull from sub-occipital muscle tension. Often from sustained flexion postures. Rapid response to manual therapy and postural correction.
Headache With Dizziness: Upper cervical dysfunction causing not only headache but vestibular symptoms. Cervical joint position sense exercises are added to the treatment program.
Post-Whiplash Headache: Cervicogenic headache following a motor vehicle accident or whiplash injury. Often the most persistent symptom. Highly responsive to manual therapy.

Cervicogenic headaches have some of the strongest evidence in PT for manual therapy efficacy. Upper cervical joint mobilization and manipulation consistently produce clinically significant headache reduction — often within the first few sessions.

Upper Cervical Manual Therapy
Targeted joint mobilization and manipulation at C0-C3, soft tissue work to the sub-occipitals and upper trapezius. Most patients experience significant headache reduction immediately.

Deep Neck Flexor Strengthening
The deep cervical stabilizers that are consistently weak in cervicogenic headache patients. Strengthening them reduces the dynamic loading on the upper cervical joints.

Postural Correction & Prevention
The workstation setup and movement habits that remove the chronic forward head loading driving the upper cervical irritation.
