Understanding the Condition
WHAT IS A ROTATOR CUFF INJURY?
The rotator cuff is a group of four muscles (supraspinatus, infraspinatus, teres minor, subscapularis) that stabilize the shoulder joint and power overhead movement. Injuries range from tendinitis and bursitis to partial and full-thickness tears — often occurring gradually from overuse or acutely from a fall or sudden force.
Rotator Cuff Tendinopathy: Degeneration and irritation of the tendon without a structural tear. Responds very well to progressive loading and manual therapy.
Rotator Cuff Bursitis: Inflammation of the bursa beneath the acromion. Often a secondary finding — the primary driver is usually impingement mechanics.
Partial Thickness Tear: Partial disruption of one or more rotator cuff tendons. Conservative PT management is the first-line treatment and is highly effective.
Full Thickness Tear: Complete rupture of a tendon. Surgical decision depends on age, activity level, functional demands, and how the shoulder responds to PT.

Typical TimelineTendinopathy and partial tears: 6–12 weeks. Full thickness tear (conservative): 3–6 months. Post-surgical: 4–6 months.
