Understanding the Condition
WHAT IS FROZEN SHOULDER?
Adhesive capsulitis — frozen shoulder — involves inflammation and thickening of the shoulder joint capsule, leading to progressive loss of range of motion and pain. It typically progresses through three stages: freezing (painful, ROM declining), frozen (stiff, less painful), and thawing (gradual return of motion). Without treatment, this process can take 18–36 months.
Freezing Stage: Gradual onset of shoulder pain and stiffness over weeks to months. Pain is often worse at night. This is the most painful stage. Manual therapy in this stage can significantly shorten the course.
Frozen Stage: Pain begins to decrease but stiffness is maximal. External rotation and overhead movement are severely limited. Aggressive manual therapy and capsular stretching are most effective here.
Thawing Stage: Motion gradually returns. PT accelerates this process significantly compared to natural history.
Secondary Frozen Shoulder: Develops after shoulder surgery, prolonged immobilization, or a rotator cuff injury. Same treatment principles but with awareness of the underlying condition.

Pain Control & Early Mobility
Grade I–II joint mobilizations to reduce pain, gentle capsular stretching, and home exercise program to maintain and gradually progress range of motion.

Aggressive Capsular Stretching
Progressive joint mobilization targeting the inferior and posterior capsule where restriction is greatest. This is the phase that drives the most significant range of motion recovery.

Strength & Full Function
Rotator cuff and scapular strengthening as range of motion is restored, progressing to full functional use of the arm.
