Understanding the Condition
ROTATOR CUFF SURGERY REHAB
Rotator cuff repair involves reattaching the torn tendon to the greater tuberosity of the humerus with suture anchors. The biology of tendon-to-bone healing is the governing factor in the rehabilitation protocol — the repair is mechanically weakest in the first 6–12 weeks, then gradually strengthens over 4–6 months.
Small-to-Medium Tear Repair: Most common repair. Conservative protocol with gradual mobilization and strengthening. Full function typically restored by 4–6 months.
Large-to-Massive Tear Repair: More complex repair requiring longer protection phase. Return of function may be more gradual and incomplete with very large tears.
SLAP Repair: Superior labral repair with biceps anchor involvement. Specific restrictions on forearm supination and biceps loading in early phases.
Revision Repair: Second surgery after failed previous repair. More conservative protocol, longer timelines, and modified exercise selection to protect the revision repair.
Treatment Approach
HOW WE TREAT IT
Rotator cuff rehab is strictly phased around tissue healing biology. We follow the surgical protocol while maximizing every phase — so when it's time to load, the shoulder is ready.

Protection Phase (0–6 weeks)
Sling use, pendulum exercises, passive range of motion by the PT. No active use of the repaired shoulder. Scapular and grip exercises begin early.

Active Motion & Early Strengthening (6–12 weeks)
Gradual active range of motion, rotator cuff reactivation, and scapular strengthening as the repair matures. Manual therapy to prevent capsular tightness.
03
Progressive Strengthening (3–6 months)
Resistance training, overhead progression, and sport or activity-specific loading toward full function restoration.
Typical TimelineReturn to daily activity: 3–4 months. Return to overhead sport: 5–7 months.
