Understanding the Condition
HIP REPLACEMENT REHAB
Total hip arthroplasty replaces the femoral head and acetabulum with prosthetic components. Modern anterior-approach procedures allow faster recovery with fewer precautions. Regardless of approach, the glutes, hip flexors, and surrounding musculature need systematic rehabilitation to restore full function.
Posterior Approach: Traditional approach with posterior hip precautions (no flexion past 90°, no internal rotation, no crossing legs) for 6–12 weeks. Protocol adherence is critical.
Anterior Approach: Newer approach through the front of the hip. Fewer precautions, faster early recovery, but anterior hip flexor sensitivity in early phases.
Revision Hip Replacement: Longer timelines, modified precautions, and a more gradual loading progression than primary replacement.
Hip Fracture With Replacement: Occurs in the context of trauma — often in older adults. May have additional considerations around bone healing and fall risk.

Early Mobility & Precautions (Weeks 1–6)
Gait training with assistive device, hip precaution education, quad and glute activation, and progressive weight-bearing as tolerated.

Glute & Hip Strengthening (Weeks 6–12)
The glutes that were inhibited by years of arthritic pain and the surgery itself need to be systematically rebuilt. This is what drives long-term function.

Return to Full Activity
Stairs, inclines, recreational activity, and the strength program that maintains hip stability and implant longevity.
Typical TimelineWalking without device: 4–8 weeks. Return to recreational activity: 3–6 months.
