Understanding the Condition
WHAT IS AN ANKLE SPRAIN?
An ankle sprain involves stretching or tearing of one or more ankle ligaments — most commonly the ATFL and CFL on the lateral side from an inversion injury. Grade determines severity, but even Grade 1 sprains require proper rehab to restore neuromuscular control and prevent recurrence.
Grade 1 — Mild: Ligament stretched, no significant tear. Minor swelling, full weight-bearing. 1–2 weeks with proper rehab. Most people skip the rehab — and then sprain it again.
Grade 2 — Moderate: Partial ligament tear. Significant swelling, pain with weight-bearing, some instability. 3–6 weeks. Neuromuscular rehab is critical at this grade.
Grade 3 — Severe: Complete ligament rupture. Significant instability, marked swelling. 6–12 weeks. Surgical evaluation if instability persists after conservative management.
Chronic Ankle Instability: History of multiple sprains on the same ankle. The ligaments have healed with laxity and the neuromuscular control was never rebuilt. Very responsive to targeted PT.

Acute Management & Restore Mobility
Reduce swelling, restore range of motion, begin weight-bearing as tolerated. Manual therapy to restore ankle joint mechanics that are often disrupted after a sprain.

Strength & Proprioception
Peroneal and intrinsic foot strengthening, single-leg balance progression, and the neuromuscular retraining that rebuilds the ankle's protective reflexes.

Return to Sport
Reactive agility, cutting, and sport-specific drills. Confidence testing before full return — because hesitation on a healed ankle creates its own injury risk.
