Core muscle strain recovery often fails for one simple reason: people return to activity before the muscle has rebuilt the capacity to handle the demands being placed on it.
The tissue may feel better, but feeling better does not always mean it is fully prepared. Understanding why the strain happened in the first place is the key to preventing the same problem from coming back.
Your abdominal muscles, especially the rectus abdominis and the internal and external obliques, are designed to control and create movement through your trunk. They help you flex your spine, rotate your torso, and resist side bending. In sports and everyday activities, they often perform these tasks quickly and under significant force.
That combination of speed and load is what commonly leads to a strain.
The Physics of an Abdominal Strain
When you throw a baseball or perform a tennis serve, your trunk rotates quickly and forcefully. The oblique muscles are responsible for generating much of that rotation, contracting powerfully to create speed.
At the same time, the muscles on the opposite side of the trunk are working eccentrically. This means they are lengthening while still producing tension to control the movement. This eccentric phase is where the muscle experiences some of its highest demands and where strains commonly occur.
Think of it like pulling on a rubber band. If you gradually stretch it, it can handle the force. If you suddenly pull it beyond its limits, it may snap. Muscles work similarly. A muscle that is fatigued, underprepared, or exposed to more force than it can handle is more likely to strain.
What “Healing” Actually Means for Muscle Tissue
After a strain occurs, your body begins repairing the damaged tissue by creating new collagen fibers. This process starts quickly, but creating strong, organized tissue takes time.
One of the biggest challenges is that the muscle often feels better before it is fully healed. Pain decreases, movement improves, and it becomes tempting to return to normal activity. However, the new tissue is still weaker and less organized than the original tissue.
Without progressive loading, the muscle does not get the stimulus it needs to rebuild strength and resilience. Returning to the same demands that caused the injury in the first place can lead to another strain.
The Role of the Nervous System
Something many people do not realize is that a muscle injury affects more than just the tissue itself. After a strain, the nervous system can reduce how strongly it activates that muscle as a protective response.
This means the muscle may heal but still not function the way it should. Other muscles may begin compensating, which can increase stress on the area and make future injury more likely.
Physical therapy helps address this by gradually restoring muscle activation and control. Early exercises may include isometric contractions, where the muscle produces force without much movement. These allow the nervous system to reconnect with the muscle while limiting stress on healing tissue.
Why Sports Require a Specific Endpoint
If your goal is returning to a rotational sport like tennis or baseball, recovery cannot stop at simply being pain-free.
Your core needs to handle the same demands your sport requires: fast movement, high force, and repeated effort under fatigue. A program that only prepares you for slow, controlled movements has not fully prepared you for competition.
This is why power training becomes an important final phase of recovery. Exercises like medicine ball rotational throws and overhead slams help retrain the nervous system and rebuild the ability to generate force quickly.
Without this step, athletes often return feeling good for a short period of time, only to re-injure the same area once they increase intensity.
Loading Is the Treatment
The surprising truth about muscle strains is that the same thing that caused the injury—mechanical load—is also what helps fix it.
The difference is the dose.
Early in recovery, the tissue needs lower levels of stress with controlled movements. Over time, you gradually increase the range of motion, resistance, and speed. This progressive loading process helps rebuild strength, improve tissue capacity, and restore the coordination needed for demanding activities.
