Understanding the Condition
WHAT IS IT BAND SYNDROME?
The iliotibial band is a thick strip of connective tissue running from the hip down the outside of the thigh to the knee. IT band syndrome occurs when this tissue becomes irritated — typically from repetitive friction at the lateral femoral condyle. It's almost always a hip strength and biomechanics problem, not a tissue problem.
Classic IT Band Syndrome: Pain on the outside of the knee that starts after a predictable distance or time. Worsens with downhill running. Often resolves with rest then returns immediately.
Hip-Driven IT Band: Weak hip abductors and glutes cause the knee to drop inward under load — increasing IT band tension. This is the most common driver and the most commonly missed.
Training Load Spike: Sudden mileage increases, new terrain, or worn-out shoes are common triggers. The tissue tolerance was exceeded before the underlying weakness was addressed.
Chronic IT Band Syndrome: Symptoms present for months despite rest and stretching. The tissue has become sensitized and the underlying biomechanics have never been addressed.
Treatment Approach

IT band syndrome responds very well to treatment — when you treat the right thing. We address hip strength, running mechanics, and tissue load simultaneously rather than just treating the painful spot.

Reduce Irritation & Restore Mechanics
Soft tissue work to the lateral hip and TFL, joint mobilization, and taping to offload the IT band while normal mechanics are restored.

Hip & Glute Strengthening
The glutes, hip abductors, and external rotators that control knee alignment under load. Weakness here is behind the vast majority of IT band cases.

Return to Running
Progressive return-to-run protocol, cadence and mechanics coaching, and load management to ensure the mileage build doesn't recreate the problem.
